Bus Transportation 2025-2026

Please fill out the information below if you are requesting bus transportation for the 2025-2026 school year.
Fill out one form per student.
Transportation services is provided for students who live within Heber School District boundaries.
Students who are attending HESD on an Inter-District do not qualify for transportation services.
